You may pre-register for admissions, labs or x-rays by visiting the hospital, or by calling 863-679-6858 between 8 a.m. and 4:30 p.m. Twenty-four hours notice is preferred.

You now have the option to pre-register online at least three business days in advance of your requested procedure date. Your information will be sent directly to our facility in a secure format. Online pre-registration is available for all inpatient and outpatient services, including diagnostic testing.

Please remember to bring the following items to your appointment:

• Photo Identification
• All applicable insurance cards
• Any orders from the doctor
