You may pre-register for admissions/labs/x-rays by visiting the hospital or calling (863) 679-6858 between 8:00 a.m. and 4:30 p.m. Twenty-four hours notice is preferred. Please bring the following items to your appointment:

• Photo Identification
• all applicable insurance cards
• any orders from the doctor
